An ID card for any type of supporting animal (a service animal ID, emotional support animal ID, or a therapy dog ID) is a little laminated thing that contains important information about the animal and its owner. The information you need to give us includes the following points:
- Your dog’s name;
- Dog’s breed;
- Name of handler/owner;
- Street address;
- City, State;

Why do I need an additional ID card?
