Description

Medical Alert Card

Emblem
Medical Alert Card

Medical Alert Card tells the people around you what to do if you have a medical emergency — and that your service dog must stay with you. Choose a plastic wallet card that sits next to your ID, or a set of 3 snap-apart mini tags for your keys, your dog’s collar and your bag.

PLEASE NOTE:
The Medical Alert Card is not an animal registration and not an ID card.
It is a personal emergency note. To register your animal, choose Service Dog Registration or Emotional Support Animal Registration.

What We Print on Your Medical Alert

Your emergency instructions
In your own words: your condition and what to do. For example: “Epilepsy. My dog stays with me. Call 911.”

Your emergency contact
A name and a phone number to call if you can’t speak for yourself.

You and your service dog
Your name and your dog’s name, so responders know the dog is with you and must not be separated from you.

Service Dog Benefits

After you click Add to Cart, we ask for two short details: what people should do if you need help, and who to call. If you order the Medical Alert separately from a registration, add your dog’s name in the order notes.

Wallet Card or 3 Mini Tags?

Wallet Card
The same plastic card size as your ID, with room for up to 4 lines of instructions. Keep it next to your ID card in an ID Card Holder, so responders find both together.

3 Mini Tags
Three snap-apart tags: one for your keys, one for your dog’s collar and one for your bag. Shorter text: up to 3 lines, 60 characters.

The Medical Alert Card helps in an emergency, but it does not register your animal and does not replace an ID card. To register your animal and get an official MyServiceAnimal ID card, choose the registration for your animal:

Service Dog Registration
Online registration with a physical ID card.

Psychiatric Service Dog Registration
For dogs trained to help with a psychiatric disability.

Emotional Support Animal Registration
For animals that provide comfort and emotional support.
